Life is good for a young cop named Rusty and his wife Rachel in a small town named Clear River, an hour outside of Philadelphia. Rusty's brother, Jeremy, is engaged to Beth at their father's trucking company, which Jeremy now runs, and Rusty is a happy-go-lucky man on top of the world. But when the Sheriff tells Rusty that the town may be under investigation as a drug hub for a huge traffic deal, things turn.
An ex-convict named Izzy makes a drug deal before hopping into a truck and driving off on the highway, toward Clear River.
Rusty sees the truck that night off the road, pulls over his car and and walks over. He finds drugs in the back and no one around. Then, a voice asks if that's you and Rusty pulls his gun on Izzy. "Is that who?" Izzy looks over Rusty's shoulder, but before Rusty can see who it is, he hears a gunshot, and he drops.
Two years later, unable to work anymore, Rusty spends his days in the local pub. His injury causes headaches so severe he needs medication. Mixed with alcohol, he becomes delusional. After seeing a school fire on the news he calls 911 to tell them that he saw a young boy trapped in a gym closet. There is no boy, and Rusty is labeled crazy.
Meanwhile, Izzy is being let out of prison from a different charge. The people that bailed him out are drug lords that beat him and tell him to fix the problem he left behind in Clear River, and to take care of Jeremy. He eventually gets on a train to Clear River, where the only other passenger is a young man named Eli from Philly.
Rusty has visions of both men getting off the train. He recognizes Izzy, and hears him say he is going to take care of Jeremy on his phone. Eli heads to a diner to eat, and Rusty then has visions of Eli meeting with his wife Rachel at the diner. As they talk, a spark is recognized.
Rusty continues to see visions of Eli with Rachel, taking long walks and talking about how Rusty was no longer the person he once was, that he lost that spark and the relationship was over a long time ago. She says the moment she met Eli she saw that spark in his eyes. They kiss. Later, Rusty accuses Rachel of having an affair. She denies it.
Rusty goes after Eli that night.
On his way, the Sheriff sees him and stops him to tell him he saved a young boy's life. Turns out there was another fire at a different school, and because of his call they checked the gym closet. A boy was there and saved. Rusty realizes his visions are true. However, he also begins to realize that he saw the future, not the present as he assumed.
When he finds Eli, he follows him to a dark park. He confronts Eli, who tells him that he is not having an affair with Rachel, that he is an FBI agent working on finding the guy in charge of this. Rusty realizes this is true. Just then, Beth shows up and tries to shoot Eli, but Rusty jumps in front of the bullet. Eli then shoots Beth dead.
With sirens and flashing lights drawing closer, Eli leans over to Rusty, who is not yet dead. Rusty tells Eli to take care of her. Tells him he can no longer make her happy, but you can. Eli is confused by this.
The final scene at the funeral, Eli walks over to tell Rachel he is sorry for her loss, and then introduces himself. On Rusty's recommendation, he says if she needs someone to talk to, they could go to the diner. She looks into his eye, sees something, and she says yes.
